GIG HARBOR FAMILY EYECARE makes every effort to keep our eyecare services easily accessible and affordable for our patients and their families. To maximize your eligibility and coverage, we work with most of the major medical insurance plans.

GIG HARBOR FAMILY EYECARE is pleased to announce that we have agreed to offer Costco Members a combined 'routine' vision service at a low warehouse price. (Invoiced as S0620 & S0621, these combined 'routine' services are not billable to or reimbursed by Medicare, Medicaid, or Private Insurance). Routine eye exams with glasses Rx $80, Contact lens exam $110-130

We offer a variety of PAYMENT OPTIONS including: CASH, CHECK, DEBIT and CREDIT CARDS.

For those warehouse members that wish to use their pre-paid vision care or medical insurance benefits instead of their COSTCO MEMBERSHIP PLAN, we also are contracted with and are preferred providers for a number of membership groups and third party payers (INSURANCE COMPANIES). We are happy to bill on your behalf and in strict accordance to the CPT* coding and billing guidelines and our insurance contracts the following payers.

A Vision Insurance plan helps with the cost of an eye exam, eye care, eye wear, or other vision related expenses. Your vision insurance plan gives you the benefit of being able to receive regular eye exams and eye care.

If you are uncertain about your benefit eligibility, you will need to check with your human resource department or call your insurance company directly and inquire about eligibility.

Questions you should ask: 

* Does my insurance plan provide optometric vision benefits and coverage?

* Can I see the eye doctor of my choice?

* What does my plan allow?

If your medical policy allows you to see any eye doctor of your choice we will verify your coverage, file the claim electronically and accept assignment of benefits for you. To verify your vision coverage, we will need a phone number for verification of benefits, a group, plan or policy number, the social security number of the insured, member and patient's date of birth, and name of employer. Verification of insurance benefits must be completed 24 hours prior to your appointment.

When you arrive for your appointment we will need a copy of your insurance plan policy card. You will be expected to pay in full the difference in the amount allowed at time of service. If for any reason the insurance company pays less than quoted you will be responsible for any amount unpaid.
